After Game Report

• Used for reporting a SRSL game.
• NOT AN INCIDENT REPORT! See Incident Report below.
• Two forms of submission (Choose only ONE):

Incident Report for Referees

• Used for explaining an incident that occurred during an SRSL game.
• Submitted by the Referee
• Two forms of submission (Choose only ONE):
• • Web-form - Window will pop up, fill out, click submit.
  • U.S. Mail - Adobe Acrobat Needed

Referee Evaluation

• Used for evaluating a Referee in the SRSL.
• Submitted by the League Members.
• Two forms of submission (Choose only ONE):
• • Web-form - Window will pop up, fill out, click submit.
  • U.S. Mail - Adobe Acrobat Needed!

Referee Score Cards

• Used for scoring an SRSL game.
• Can be submitted by the Referee as proof of game outcome.
• Double Sided - Tips are located on back of cards.
• One form only: PDF Format - Adobe Acrobat needed!

Field Evaluation

• Used for evaluating a Field in the SRSL.
• Submitted by the League Members.
• Two forms of submission (Choose only ONE):
• • E-Mail - Window will pop up, fill out, click submit.
  • U.S. Mail (Coming Soon)

Request for Meeting

• Used for requesting an SRSL Meeting
• Can be submitted by League Members
• One form only: U.S. Mail - Adobe Acrobat Needed! (Coming Soon)
